mettre en place, gérer et assurer le fonctionnement d'une structure d'hospitalisation à domicile, alternative à l'hospitalisation mentionnée à l'article R.6121-4 du code de la santé publique; cette structure sanitaire permet d'assurer au domicile du malade, pour une période limitée mais révisable en fonction de l'évolution de son état de santé, des soins médicaux et paramédicaux continus et nécessairement coordonnés; ces soins se différencient de ceux habituellement dispensés à domicile par la complexité et la fréquence des actes; dispenser des soins palliatifs à domicile; mettre en oeuvre un programme d'actions sanitaires et sociales; former les professionnels intervenant à domicile dans le cadre des services d'hospitalisation à domicile à des soins techniques particuliers ou la prise en charge de la douleur ou des soins palliatifs
